New Patients*

If you are a new patient and would like to schedule an appointment with a doctor, nurse practitioner, or with one of our Specialists, please call our Clinic Operator at 1(800) 4 SANSUM or (800) 472-6786. Our appointment schedulers will be able to match you with a physician or other provider at the location where you will receive your care.

If you are a new patient and would like to schedule an appointment at one of our Distinguished Specialty Centers, please call that center directly.

New patients with a Physician Referral, please call our Clinic Operator at 1(800) 4 SANSUM or (800) 472-6786. Our appointment schedulers will be able to connect you to the department or specialist to which you are being referred.

New patients to our Primary Care Department can call (805) 692-4650 to schedule their first appointment. One of our Patient Outreach Coordinators will help with the selection of the new primary care provider and walk you through the new patient process. 

Previous medical records can be emailed to outsiderecords@sansumclinic.org.

New patients may print and complete our Registration Forms* (in English or Spanish) before their visit to help speed check-in at Registration.

Appointment Reminders Via Text

Patients now have the option to receive appointment reminders via text message!

Simply text the word SANSUM to 622622. You will received a text reply to verify that you are signed up. To cancel this service via text, send a text that says STOP to 622622 and your appointment will be cancelled and removed from our schedule!

Same Day Appointments

Several of our primary care locations offer same day appointments. We know that you can’t always anticipate an illness or injury and it can be difficult to get an appointment at the last minute. Same day appointments enable us to care for you when unplanned conditions occur. For same day appointments, we will make every effort to schedule appointments for serious illnesses as soon as possible. Appointments are prioritized for minor yet urgent problems.

Schedule Same Day Appointments for:

  • Cold and Flu Symptoms
  • Sore Throat
  • Rash
  • Earache
  • Cough
  • Urinary Tract Infection
  • Diarrhea
  • Nausea
  • Asthma
  • Allergic Reactions

Please note: Same day appointments are not for treating long-term chronic issues or preventive healthcare (for example, they are not for diabetic checkups, medication refills, general exams, pap smears, well child exams, or immunizations.) They are also not for medical emergencies. If you or a loved one experiences a medical emergency, such as chest pain, stroke, difficulty breathing, serious injury or trauma, please visit the nearest emergency room or call 911.
